Immunization Requirements


 
All newly admitted incoming UCLA students must provide documentation of vaccination or immunity to certain infectious diseases before Sept 19, 2016.  In addition, newly admitted incoming students at high risk for tuberculosis must provide documentation that they have been screened for tuberculosis within 6 months of entry to UCLA.

You can satisfy these immunization requirements​​​ through the Ashe Secure Portal. 

  • Submit your Statement of Intent to Register (SIR/SLR).
  • As part of the SIR/SLR process, you will receive a UCLA log-on ID.  Please allow 2 business days after submitting SIR/SLR before attempting to log into the Ashe Secure Portal
  • Log into the Ashe Secure Portal
  • Select “Forms” from the options on the left side of the page.
  • Complete the forms labeled  “Immunization Compliance / Health Clearance” & “TB Screening”
  • Enter your immunization history where prompted.
  • Upload a copy of the supporting documentation (yellow card/ vaccination records, blood tests, or letters from your health care provider). If you need a form to bring to your health care provider, please click here​
