2017-2018 Voluntary Enrollment Information

Non registered students enrolled in the Voluntary UC SHIP Plan will have the same benefits as registered UC SHIP students and must follow the same policy and referral requirements. Students must come to the Ashe Center for their medical needs and a referral will be written if it is medically necessary and we cannot provide the medical treatment. Non registered students purchasing the voluntary plan will continue to have their behavioral health services through UCLA Counseling and Psychological Services (CAPS) and UCLA Behavioral Health Services (BHS) with a CAPS referral. Non-Registered students and dependents are not automatically enrolled. Dependents of qualified non-registered students also have the opportunity to enroll in the voluntary plan.
 
The following individuals may enroll voluntarily for UC SHIP:
  • All former graduate students (undergraduate & graduate) of UCLA who have completed their degree (graduated) during the term immediately preceding the term for which they want to purchase coverage. These individuals may purchase plan coverage through Wells Fargo Insurance Services at (800) 853-5899, for a maximum of one semester or one quarter and must have been enrolled in the plan in the preceding term.

 

  • All non-registered graduate “Filing Fee Status” students of UCLA who are completing work under the auspices of the University of California, but are not attending classes. Students on Filing Fee status must purchase UC SHIP through the Wells Fargo Insurance Services at (800)-853-5899. Filing Fee students are allowed to purchase UC SHIP for a maximum of one semester/quarter. The student must have been covered by the plan in the term immediately preceding the term the student wants to purchase, or, if the student waived enrollment in the prior coverage period, show proof of loss of the plan used to waive. Proof of loss means an official letter of termination from the insurance carrier.

 

  •  All non-registered graduate students of UCLA who are on a planned educational leave or approved leave of absence may purchase plan coverage of a maximum of one semester or two quarters. These students must purchase UC SHIP through Wells Fargo Insurance Services at (800) 853-5899. The student must have been covered by the plan in the term immediately preceding the term the student wants to purchase, or, if the student waived enrollment in the prior coverage period, show proof of loss of the plan used to waive. Proof of loss means an official letter of termination from the insurance carrier.

  • ​Concurrent Status
    Undergraduate students who only have one more class to complete their degree, are in the process of completing it through the UCLA Extension Concurrent Enrollment Program and are not matriculating as a full-time UCLA student are eligible to purchase and enroll as a voluntary concurrent status enrollment for a maximum of 1 term.

Enrollment in the voluntary plan is managed by Wells Fargo (on behalf of Anthem Blue Cross) and the cost of insurance premium is paid directly to Wells Fargo. Student Health Services DOES NOT manage the enrollment or collect payments for the voluntary plan.
