I need to have my hospital/provider bill reconsidered. Who do I contact about my claim(s)?

    Please submit your appeal to Anthem Blue Cross by calling the Member Services line at (866) 940-8306. If you cannot reach them, please email the Student Health Insurance Office at ASHEINSUR@ashe.ucla.edu, stating the surrounding circumstances of your claim(s) and include your student ID number for security purposes. If deemed appropriate and necessary, the office will contact Anthem Blue Cross on your behalf for processing.
