There isn’t a specific clinician name listed on my immunization card – how do I complete that section of the requirement?
When prompted to enter the contact information for the clinician/facility where you received your shot, you should use the information for the provider/facility that will be able to verify that you’ve received your shot series. Most students submit their most recent or current healthcare provider name and address. However, you may also use the facility name e.g., Hospital X, Care Clinic X, High School X, Public Health Facility X, etc…, where you received the shot.
