California law requires students entering the University of California, who are 18 or younger, to provide proof of immunity to Hepatitis B prior to enrolling. The University Hepatitis B Requirement is a condition of enrollment here at UCLA and is strictly electronic; no paper documentation is required. Students should review the requirement details as soon as they send in their Statement of Intent to Register (SIR) to the University (usually in April); the Hepatitis B Requirement is listed as one of the “Priority One” items in the New Student Guide for Freshmen. Students should complete the online questionnaire and submit their information to the Ashe Center Hep B Requirement Portal by the beginning of their first quarter at UCLA (September/beginning of Fall Quarter). Please note, to log onto the
Hep B Requirement Portal, you will need to use your UCLA log on id and password.
The University Hepatitis B Requirement applies to most incoming undergraduate freshmen. Graduate students – check with your program coordinator for specific health clearance requirements as they differ by program.
The University recognizes that some students may have not yet completed the series or may have overlooked the requirement – students are given a two quarter grace period to satisfy the requirement. Failure to comply with the requirement after this two-quarter grace period will result in a hold being placed on your records (usually at the beginning of Spring Quarter). Don’t wait until the last minute – the process is straightforward and takes about 10 minutes to complete.
There are several ways to meet this requirement - ALL of them require that you complete an online questionnaire and electronically submit your information to our Hep B Requirement Portal. Click on the following categories to view detailed descriptions for each of the 4 ways to meet this requirement.

You think you’ve received the three shot Hepatitis B immunization series in your lifetime, but aren’t sure/can’t locate your immunization card/the information listed on your immunization card is unclear/can’t find your immunization card.
Info You Need to Provide: Proof of immunity through a blood test (Antibody Titer for Hepatitis B); the date of your titer acts as your series completion date.
It happens! It's likely that you have had the three shot Hep B series - you should first check with your high school as they tend to keep such records. If you cannot locate the information, you will need to have a blood test called an “Antibody Titer for Hepatitis B” which will confirm your proof of immunity and satisfy the requirement. The date of your titer would then serve as your series completion date when you log onto the Hep B Requirement Portal and submit your information. Log onto the Hep B Requirement Portal using your UCLA log on id and password and follow the prompts. After completing the short online questionnaire, click “PROCEED” and you will be asked to enter the contact information for the clinician/facility where you received your Hep B immunization series and your series completion date (date of your third Hep B shot). After clicking “SUBMIT” you will receive a confirmation number to indicate your compliance with the requirement and a separate confirmation email will be sent to the email address listed on your University Records System Access (URSA) account.
