Health Clearance Assessments

What to Expect

Health Clearance Assessments are conducted by Registered Nurses (RNs).

This appointment is for:

• Reviewing health history and immunizations
• Completing clearance forms for school, work, or volunteer programs

Types of Health Clearance Assessments

Scheduled Appointments (In-Person or Video)

All other types of health clearances (e.g., for graduate programs, employment, or non-UCLA volunteer roles) can be scheduled through MyStudentChart.

Note: If your clearance paperwork requires a physical exam, ADHD evaluation, or sports physical, please schedule an appointment with your Primary Care Provider (PCP), as these services are not included in the Health Clearance Assessment appointment.

Not Provided at The Ashe Center

DOT, CMV & Immigration Exams

• Patients who need a Department of Transportation (DOT) medical exam and commercial motor vehicle (CMV) certification must be seen by a licensed “medical examiner” listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. Find a medical examiner who is certified by the FMCSA to perform DOT physical exams.
• Patients who need an immigration medical exam, find an approved doctor near you.

These exams are not offered through the Health Clearance Clinic.

• Complete the Clinical Questionnaire
• Arrive 15 minutes before your appointment time
• Plan for your visit to take one hour
• There is a $20 fee for missed appointments and late cancellation (less than 24 hours before appointment time)
